The question behind the work

When program requirements call for documentation, can the organization independently demonstrate what occurred—and what happened to the participant?

HAA's approach connects applicable requirements with the records actually available. It separates what the evidence establishes from what remains missing, inconsistent, disputed, or unresolved. The purpose is practical improvement—not a predetermined finding.

Four connected capabilities

One documentation-to-improvement workflow, tailored to a clearly defined assignment.

01 / Verify

Documentation verification

Compare applicable requirements with authorized participant files, housing plans, service records, notices, and administrative decisions. Identify missing, inconsistent, or unresolved evidence.

02 / Trace

Housing continuity

Review how the record documents participant status, housing transitions, referrals, provider changes, contact, and exit planning. Flag gaps for operational follow-up.

03 / Prepare

Monitoring readiness

Organize available evidence against an agreed framework and identify documentation that may be needed to substantiate program activities during monitoring.

04 / Improve

Process improvement

Develop practical verification checklists, responsibility matrices, recordkeeping workflows, and prioritized recommendations.

Start with a focused pilot.

A defined question. An agreed record sample. A usable decision document.

How an engagement works

Limited, transparent, and designed around the client's documented need.

01 / DEFINE

Agree on the question

Confirm the program, applicable authorities, record sample, deliverables, and conflicts before engagement.

02 / RECONCILE

Compare requirements and evidence

Identify supported facts, documentation gaps, limitations, and follow-up questions.

03 / IMPROVE

Provide an action roadmap

Deliver concise written findings and practical process recommendations, then present the results virtually.

Independence and information handling

HAA's public-interest research and prospective paid technical-assistance engagements have different purposes. Potential work is screened for conflicts before acceptance; HAA does not offer paid reviews of the same matters in which its founder is personally involved. Any client engagement requires appropriate confidentiality, records-access, and information-handling arrangements. Confidential client materials will not be repurposed for public reporting without an appropriate legal basis and authorization.

Initial inquiries should describe the operational problem without attaching participant records, medical information, or other sensitive material. HAA does not provide legal advice, official compliance determinations, or guaranteed monitoring outcomes.
